About Your Visit |

Please bring the following items with you at your appointment:
- Insurance Card and photo ID
- Any medical records that you wish for the doctor to review
- A list of all current medications and dosages

New Patient Appointment |
When you arrive to our office you will enter your medical information on a computer. Please arrive early so you will have time to complete the electronic medical record forms that allow us to provide the best of care. Our friendly staff is here to help you if needed. |

YOUR Electronic Medical Record (EMR)
Modernizing Medicine, has been selected by Shea ENT and Eubank Audiology to provide you, our valued patient, with a custom electronic medical record. This Electronic Medical Record company has been certified HIPAA compliant (which means your medical information is protected). Your prescriptions are "E-prescribed": your medicine can be waiting for you at the pharmacy of your choice. Your current problem, your past medical history, family history, allergies, medications and review of multiple system problems can now be recorded by YOU, and not transcribed from a paper form.
With all of this "hi-tech" health care, we have proudly retained "hi-touch" Texas friendly. You may notice several "y-alls" from our home-grown office. |
